THIS IS A GOOD REVIEW OF PROGRESSIVE RECOMMENDED TREATMENTS FOR ACUTE AND DECOMPENSATING

This situation should be RARE with a well-diagnosed and well-treated patient. 
The problem, of decades duration, continues to be the failure of Pulmonologists to have their patients EVALUATED AND TREATED for
concomitant BRONCHIAL ASTHMA - a combination that exists up to 30% of the time.
